Soccer Youth Team Season Application 2008/2009

Please fill out the application below. A non-refundable $300.00 deposit is due with the application.  For online applications, we must receive the deposit within 3 days of notification of league acceptance.  If we do not receive the deposit within 3 days, your spot will no longer be held in the league - NO EXCEPTIONS.  Submit the completed application below and mail payment to The Sports Zone, 29 Trefoil Drive, Trumbull, CT 06611. If paying by check or money order, please make payable to The Sports Zone.  If paying by credit card, it will be necessary to come to The Sports Zone to swipe your card.  We do not take credit card deposits via the phone.  Thank you.  For team fees and additional information, please refer to the soccer page under the appropriate sections: Youth Rates.  Please note that Teams that fail to submit the full payment and completed forms will not be permitted to play. 

In addition, a $3 per person fee for CJSA insurance is required for all youth players. This per person fee is  valid for the entire winter season. Teams: Please make one separate check out to The Sports Zone and submit along with your team roster and waiver form to The Sports Zone staff.   Check with us for your appropriate due date.
